[MD setting] minor UI touch ups from Alan

Alan details some UI fixes as Pri-3 importance. This bug covers the items in that Pri-3 list.

- move dialogs closer to harmony spec. New spec

- What can we do about favicons? If we can’t reliably display favicons, I want to remove them from the design altogether. How does myactivity.google.com do it? And Flash-Blocked?
