I can reproduce this in non-MD (ash-md set to 'Disabled' in about:flags) in the latest canary 57.0.2978.0 so I don't believe this to be a regression caused by material design. Targeting for M-57. Mohsen, mind taking a look? Note, the bug does *not* reproduce (i.e., the headphones icon appears as intended) in the following situations: * Plug in the headphones first and then open the system menu. * After you have successfully seen the headphones icon appear once, plugging the headphones in/out while the system menu is open seems to show/hide the icon. Additional note: When inserting/removing the headphones, the thumb of the volume slider sometimes shifts position.

Regarding "Additional note" in #4, Ben points out this is likely because we cache separate volume states per device, so disregard that point for the purposes of this bug.
