Data reduction proxy should have separate files for request and response headers |

Issue description
The data reduction proxy currently has data_reduction_proxy_headers.{h,cc}. This file historically only pertained to information in responses, but now includes information about requests. For example, the method chrome_proxy_accept_transform_header(), returns the name of associated request header.
Instead, the headers should be split into two files, one for request headers, and another for response headers.
