Interesting. It seems the heuristics are thrown off because there a two fields between the credit card name and cvc, and the cardholder's name, and expiration.

The heuristics expect the credit card fields to be one after the other, and then checks if a minimum set of fields have been found. This happens for the second part, but not the first.
