3. How do you get to that form page? What's the full URL?

I think form data save (autocomplete) is broken pretty recently. I will look at it in the next a few days.

It is not a very recent problem. As I stated above 55.0.2883.45 and 54.0.2840.85 both have this issue. 46.0.2490.76 - works fine.
