Unfortunately I could not find specific request that caused this to fail and therefore can't investigate it further. We have a low percentage of errors - less than 0.1% of total, so we recommend to just refresh if it doesn't work first time. Having said that I have just today enabled monitoring for increase in numbers of errors, so hopefully we'll be able to fix these issues faster.
