reverting in gerrit should make it easier to DTRT

Issue description

i've explained to many people over the years (and continue doing so) that you have to fill in details when reverting a commit.  just hitting the "Revert" and "Submit" buttons is not sufficient.

can we make this easier in gerrit ?  here's the ideal workflow:
- when hitting Revert, the generated commit message will preserve all BUG= and Bug: lines
- the UI that comes up should include two text fields: the first one includes the "revert xxx" message that is generated today, and the second field is left blank -- but gerrit won't let you revert until you enter some information into that as to *why* the CL is being reverted.

This is done -- the revert button now prompts for a reason and preserves various footers.
