It should link to the github repo at the given commit.  e.g. https://github.com/luci/luci-py/commit/bae452c

I think the swarming-app toolbar may be a good spot for it, perhaps near the sign in bar?

yes but I'd also recommend to reduce the size of the user email address and sign in/sign out links to be significantly smaller; I'd go as far as to reduce the size of these two to 60% of their current size, so that adding the version would still fit without overwhelming. Otherwise it could be added on a hover/drop down but I'd prefer to not have to go that path.
