Headers should be stored in protobuf maps instead of as a list of strings

Then we could do vary comparisons directly on the protobuf objects instead of first copying them into ServiceWorkerFetchRequest and ServiceWorkerResponse objects.

This is low priority as presumably the vary check usually passes anyway.
