Add ability for admin to hide extension icon by default |

Issue descriptionIn some cases, it is desirable to force-install extensions and have the icon hidden by default, to reduce the toolbar clutter for users. For example, certificate installation extensions have no need for a toolbar icon, and could be hidden by default.
