Videoelement not able to dispose mp4 references correctly after removal from DOM

Reported by mircomeu...@gmail.com, Aug 18 2016

Issue description

UserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_11_6) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/52.0.2743.116 Safari/537.36

Example URL:
http://codepen.io/anon/pen/qNwEOj

Steps to reproduce the problem:
1. create video instance and set src to some mp4 (all on same domain)
2. remove the video from the dom
3. create new video instance and set src again with a different url but from the same domain

4. ...redo step 1 - 3 round about 8 times

What is the expected behavior?
I expect no influences on memory or held connections from already removed elements.

What went wrong?
Video is not able to show

Did this work before? No

Is it a problem with Flash or HTML5? HTML5

Does this work in other browsers? Yes

Chrome version: 52.0.2743.116 Channel: stable
OS Version: OS X 10.11.6
Flash Version: Shockwave Flash 22.0 r0

This is per-spec, you need to clear src but setting it to src='' sadly. We close the ones we can if we detect they're idle, but this can't always be done.
