Better "top hosts" cutoff

The current algorithm is simply top 20. Have two additional thresholds:
1. # of visits in the last month
2. % of visits in the last month

#1 helps avoid data usage for light users of the browser.
#2 helps avoid data usage for sites that are less likely to be revisited.

Thanks for opening this bug.

In addition we should also explore a time-weighted visit percentage scheme.
For example, below code gives 3x weight for todays visits, 2x for a week ago, 1.5x for three weeks ago, etc.

Note that if global ranking (https://bugs.chromium.org/p/chromium/issues/detail?id=654166) becomes the default, then that would obviate this bug.
