pdf filled with form fields and checkbox, does'nt render checkbox tick when view within chrome browser

OS Version: 6.1 (Windows 7, Windows Server 2008 R2)

     Safari 5:

What steps will reproduce the problem?
1. open attach pdf file in chrome. the checkbox is not ticked.
2. download the pdf and view with adobe reader the checkbox tick is shown
3.

What is the expected result?
when pdf viewed within chrome browser, it should show all the fields and checkbox tick.

What happens instead of that?
checkbox tick/selected not shown.

Please provide any additional information below. Attach a screenshot if
possible.
itext-pdf  version 5 is used to fill form fields and data in pdf.

Unable to reproduce the issue on win8.1 chrome version 52.0.2743.82 and canary 54.0.2810.2 - No checkbox is ticked in chrome and in adobe reader view as well

No tick is seen in firefox browser also

Could you please upgrade to latest stable and let us know if issue still exists.

I see 3 checkboxes in the left column under "Rechnungsversand" and 4 in the right column under "Klima Option" - same with Acrobat Reader.

If this is still a problem, please include a screenshot of what the PDF looks like on your computer when viewed with Chrome.
