Voiceover cursor doesn't synchronized with the system focus when tab is used

Issue description

Chrome Canary 53.0.2783.4
Mac OS X 10.11
On some websites, after you tab around and then try to use the Voiceover navigation keys Ctrl-Option-Left/Right, you observe that the Voiceover cursor jumps to an unrelated location like the top of the page. It seems that in some cases the Voiceover cursor doesn't follow the system focus, even though the Voiceover setting "Track cursor" is correctly enabled.

Could you try and do some initial testing to see where the cause is?
To repro, open up a bug in Google's internal bug tracking system, tab to the comment edit field and then tab once more. After that try using the Voiceover navigation keys and see where the focus lands. It jumps to the top of the page for me.

For what it's worth I think this issue affects us as well.  We've noticed it in some complicated Polymer apps and suspect it might be related to the shadow dom.

Unfortunately we haven't been able to create a simple reproduction.
