Failing to play audio in both HTML5/Flash

Issue description

Any webpage that contains html5 audio or attempts to play audio via flash player simply does not play. I also checked youtube, it's plays video but with audio muted.

I don't know exactly how to reproduce this issue, it happens randomly but occurred several times. 

btw: this issue can be temporarily fixed by restarting Chrome.

Unable to reproduce the issue on Mac OS 10.11.5 using chrome latest stable M51-51.0.2704.103. Able to play youtube videos with no issues.

Is this issue seen on incognito mode as well? Please do recheck this issue by creating a new profile under chrome://settings with no apps or extensions in your browser.

Can you check if you have Pepper Flash (version of Flash that you can find in Chrome://plugins, after expanding, then looking on the line under Flash that says Location) and if so, disable it. Then check your audio.

If it doesn't, please download another version of Flash through Adobe, which you can do so here: http://get.adobe.com/flashplayer/?fpchrome

Another area to check is your graphics card driver, to ensure that it is up to date.
