Have per-document user gesture unlock for media instead of per-element

Rationale is that it is easy to use one user gesture and "unlock" many media elements.

Disclaimer: it doesn't mean it will be implemented but to be considered.

This would reduce the need to do silly things in order to support playlists, so I think it's well worth exploring. The risk is of course that there are currently media elements that are being blocked (perhaps never intended to really play) that would now start playing, but that can only be learned by shipping I think.
