Dotted line focus rectangle with Material Design top chrome

Issue description

Version: 53.0.2767.4 / ToT
OS: Linux, Chrome OS

What steps will reproduce the problem?
(1) Press Alt+Shift+T (focus toolbar)
(2) Use Tab key to switch focus to views in chrome toolbar and location bar

What is the expected output?
No dotted line border. Ink hover used to indicate focus.

What do you see instead?
Some views (Back button, service handler icon) still use black dotted focus rectangle.

The back button is already fixed.  Content settings icons use a dashed rect rather than an ink hover, so they haven't been converted yet; I think  bug 591140  covers this.  Duping.

I did find that the star and content settings icons have no hover state, which seems wrong, and I couldn't find a bug to cover that.  Evan, are you aware of a bug here?
