Unable to add new address in chrome://md-settings/autofill

Issue description

Chrome Version: 53.0.2773.0 ced2fcee2c85702055d028f4e3e48c5a75a7e41c-refs/heads/master@{#400610} 32/64-bit.
OS: Windows(7,8,10), Mac (10.10.5)(10.11.4), Linux(Ubuntu 14.04 LTS) 

What steps will reproduce the problem?
1. Launch chrome and navigate to chrome://md-settings/autofill
2. Now click on 'Add Address' and observe

Actual : Unable to add new address in auto fill settings
Expected : Should be able to add new address in auto fill settings

This is a non-regression issue, seen from M-52 series as 'Manage profile' option appears from build # 52.0.2740.0

Correction : This is a non-regression issue, seen from M-52 series as 'Add Address' option appears from build # 52.0.2740.0

Status: Untriaged (was: Unconfirmed)
Able to reproduce the issue on mac 10.11 chrome version 53.0.2773.0

Confirming the issue for more inputs on this
