cidb: Add CQ cl_actions counter metric

This adds a Monarch counter for cl_actions. There is a separate metric
per action type, with "reason" being a field. This was done because the
number of possible reasons is very large, so we don't want to overload
the number of field tuples by also putting the action type in the fields.
