While reviewing https://codereview.chromium.org/1977863003/ , rdsmith pointed out that there's cases where OpenEntry can fail, but its not recorded at all in the OpenResult histogram (e.g. the stream 0 EOF record has an implausible length).

This is really scary; we should be tracking all cases for regression. Go over all exit cases and ensure they are reporting.
