I'm unable to reproduce this on Windows 8.1 or Windows 10 (Fast-ring preview builds). This is with Canary 52.0.2735.0 or Chrome dev builds.

Odd, I'm now able to reproduce this. It now reproduces even without unchecking "Send system information" and clicking the Send button. Just select Report an issue from the Help menu directly, close the dialog, and then select it again from the Help menu. Wait a moment after it shows and it will then disappear. The Render process is being reported as dying, which kills the dialog (which is an extension itself).

The latest Canary (53.0.2750.0) doesn't seem to exhibit this behavior. At least when I follow the original steps.
