Expose disabled extensions in Telemetry logs

In  Issue 605707  and https://build.chromium.org/p/chromium.gpu.fyi/builders/Android%20Debug%20%28Nexus%209%29/builds/165/steps/webgl_conformance_tests/logs/stdio (stdout attached), GL_EXT_disjoint_timer_query is disabled due to bugs, but that fact isn't recorded anywhere in the about:gpu-like dump at the beginning of the log.

Specifically, the disabled extensions should be reported alongside the driver bug workarounds.

Summary: Expose disabled extensions in Telemetry logs (was: Expose disabled extensions in about:gpu and Telemetry logs)
After checking: the disabled extensions are reported in about:gpu. They should be exposed in the GPU info fetched via Telemetry.
