Chrome Autofill populating incorrect Word Press Field

UserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_10_5) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/49.0.2623.87 Safari/537.36

Steps to reproduce the problem:
1. Fill in Word Press form with First, Middle, Last Name
2. Fill in First and Last name and select when Autofill option comes up
3. Middle name will autopopulate state of your chrome autofill

What is the expected behavior?
Nothing should be autofilled in the Middle name, other than middle name.

What went wrong?
The middle name field is getting autopopulated with the state of the users chrome autofill.

Did this work before? Yes Prior to 3/22/16 for a year

Chrome version: 49.0.2623.87  Channel: n/a
OS Version: OS X 10.10.5
Flash Version: Shockwave Flash 21.0 r0

ross.novotny@ - Could you please let us know from where can we see the First, Middle, Last Name field in the wodpress.com. While navigating to my profile I could see only first and last name field as shown in the snapshot.
