AuthType basic AuthName not displayed in prompt

Reported by mikegl...@kikflo.com, Mar 14 2016

Issue description

UserAgent: Mozilla/5.0 (Windows NT 10.0; WOW64; rv:45.0) Gecko/20100101 Firefox/45.0

Steps to reproduce the problem:
1. .htaccess file contains
 ErrorDocument 401 default
 <Files admin.php>
 AuthType basic
 AuthName "This text is not displayed in the prompt window on Chrome but is displayed on other browsers."
 AuthBasicProvider file
 AuthUserFile /home/user/htpasswd-file
 Require valid-user
 </Files>
2. Going to /admin.php causes a prompt to pop up for login information.

What is the expected behavior?
The AuthName text should be in the prompt.

What went wrong?
The AuthName text is not in the prompt. Only default text is displayed. See attached image.

Did this work before? Yes AuthName was displayed in Chrome just a few versions ago. Probably 45 to 47

Chrome version: 49.0.2623.87  Channel: stable
OS Version: 10.0
Flash Version: Shockwave Flash 21.0 r0
