Tabs can be lost entirely by dragging tab upwards too quickly

Issue description

- create a window with a bunch of tabs
- grab one and drag it up and let go of the mouse very quickly (timing is 
important, this isn't 100% repro)

expected result:
- new window with that tab

actual result:
- sometimes the new window will not appear and the tabs in the originating 
window will have a gap

The fact that there's a gap (which doesn't go away on adding a new tab) means that the 
tab strip still thinks there's a placeholder tab there. That may be a hint as to what 
particular cleanup codepath isn't getting called correctly due to the timing.
Note the windows are still in the window menu, so they haven't gone away, they're

I just saw something like this too.  An easy way to repro is to grab a tab and slide it 
to the side displacing at least one existing tab, then move it up or down a few pixels 
and let go of the mouse.  The window list shows the phantom window!
